HER2 status
1. HER2는 항 HER2 치료의 표적입니다. Adjuvant trastuzumab therapy는 유방암으로 인한 사망의 상당한 위험과 관련이 있는 distant recurrence 위험을 1/3 이상 줄입니다.
2. HER2 상태는 ㉮protein overexpression에 대한 IHC staining 또는 ㉯gene amplification에 대한 fluorescent in situ hybridization (FISH)에 의해 결정됩니다. IHC staining은 scale of 0–3+으로 표현하며 3+는 양성으로 간주되는 반면, 0-1+는 음성으로 간주됩니다. 2+ staining이 있는 경우 reflex FISH analysis가 권고됩니다. FISH는 초기 평가로 사용되거나 IHC 2+의 경우에 추가 평가로 사용될 수 있습니다. FISH 결과는 chromosome 17에 있는 centromere signal에 대한 HER2 비율이 ≥ 2.0이면 양성으로 간주됩니다. IHC가 3+ 또는 0-1+ 인 경우는 FISH를 시행 할 이유가 없으며 FISH가 ≥ 2.0인 경우도 IHC testing을 할 이유가 없습니다.
