알레르기 비염의 전형적인 증상/징후, 치료, Allergic rhinitis, typical symptoms and signs, treatments

Allergic rhinitis의 전형적인 증상/징후는 sneezing, rhinorrhea, nasal obstruction, nasal itching이므로 지문의 환자는 알레르기 비염입니다. Glucocorticoid nasal sprays가 allergic rhinitis의 가장 강력한 single-agent maintenance therapy이고 권고 용량에서 부작용이 거의 없습니다. 이것은 또한 코막힘을 경감하는데도 효과적입니다. First-generation sedating antihistamines은 처방 없이 환자가 복용할 수 있는 약제이지만 진정, 인지기능 손상, 고령에서 anticholinergic side effects, 소아에서 paradoxical agitation과 같은 부작용이 있습니다. Second-generation antihistamines은 이와 같은 문제들이 거의 없어서 알레르기 비염에서 선호됩니다.

Mild 또는 intermittent 증상인 경우 glucocorticoid nasal spray를 규칙적으로 또는 as needed로 사용할 것을 제안합니다. 최대 용량으로 시작하고 일단 증상이 조절되면 가장 효과적인 최소 용량으로 tapering합니다. Mild symptoms인 일부 환자들은 경구 투여를 원하거나 글루코코르티코이드를 피하려고 다른 약제를 선호합니다. ㉠ Azelastine 또는 olopatadine과 같은 antihistamine nasal spray를 규칙적으로 또는 as needed로 사용합니다. ㉡ Second-generation oral antihistamine을 규칙적으로 또는 as needed로 사용합니다. ㉢ Cromolyn nasal spray를 규칙적으로 또는 as needed로 사용합니다. 이것은 다른 약제보다 덜 효과적이더라도 우수한 안전성으로 소아에서 부모에 의해 종종 선호됩니다. Persistent 또는 moderate-to-severe 증상의 경우 1차 치료로 glucocorticoid nasal sprays를 권고합니다. 최대 용량으로 시작하고 일단 증상이 조절되면 가장 효과적인 최소 용량으로 tapering합니다. 만일 glucocorticoid nasal sprays 단독으로 증상을 조절하기에 충분하지 않다면 다른 약제보다 우선하여 antihistamine nasal spray를 추가할 것을 제안합니다. 또 다른 선택은 oral antihistamine/decongestant combination을 추가하는 것입니다.

! Classification of allergic rhinitis

1. "Intermittent" means that the symptoms are present:

Less than four days a week

Or for less than four weeks

2. "Persistent" means that the symptoms are present:

More than four days a week

And for more than four weeks

3. "Mild" means that none of the following items are present:

Sleep disturbance

Impairment of daily activities, leisure, and/or sport

Impairment of school or work

Troublesome symptoms

4. "Moderate-severe" means that one or more of the following items are present:

Sleep disturbance

Impairment of daily activities, leisure, and/or sport

Impairment of school or work

Troublesome symptoms
