DOAC을 복용 중인 경우 용종절제술을 계획할 때

 

Low/moderate bleeding risk surgery의 경우 시술 전날 DOAC을 중단하고 출혈이 없다면 시술 하루 후 (약 24시간) 개시합니다. 총 중단 기간은 2일입니다.

 

High bleeding risk surgery의 경우 시술 전 2일 동안 DOAC을 중단하고 (약 48시간) 출혈이 없다면 시술 2일 후 (약 48시간) 개시합니다. 총 중단 기간은 4일입니다. 일부의 경우 1일 더 추가로 기다려서 개시하기도 합니다.

 

신기능 저하가 있는 경우 apixaban, edoxaban, rivaroxaban의 경우 일정 조정이 추가로 필요하지 않지만 dabigatran의 경우 신기능이 CrCl <30-50인 경우 low/moderate bleeding risk procedures일 때는 시술 전 하루 더 추가적으로 중단하고 high bleeding risk procedures일 때는 시술 전 2일 더 추가적으로 중단합니다.

This strategy applies to all DOACs in individuals with normal kidney function (eg, CrCl >50 mL/min) and individuals taking apixaban, edoxaban, or rivaroxaban with CrCl 30 to 50 mL/min. For individuals taking dabigatran who have CrCl of 30 to 50 mL/min, omit an additional dose before the procedure. For any DOAC and a high bleeding risk procedure, it may be reasonable to omit the DOAC for an additional postoperative day (5 days total interruption). Refer to UpToDate for additional details.

정리)

1. 용종절제술은 high risk procedure이다.

2. Apixaban, edoxaban, rivaroxaban을 복용 중인 경우 신기능 여부와 상관 없이 시술 전 2일, 시술 후 1일 (총 5일) 중단한다.

3. Dabigatran의 경우 신기능이 정상이라면 시술 전 2일, 시술 후 1일 (총 5일) 중단하지만 <80보다 낮다면 그 수치에 따라 중단 기간을 연장합니다.
